ADMISSIONS
The following information will be helpful when you are considering Vera Lloyd Presbyterian Home for a client.

Program Areas Include:
Residential Long-Term Program
• 17 bed total capacity (Male or Female)
• Males 10 to 18 years of age & Females 6 to 18 years of age at admission
• Must be able to attend public school system
Shelter Short-Term Program
• 7 bed total capacity (Male or Female)
• Males 10 to 18 years of age & Females 6 to 18 years of age at admission
• Contract is for 45 days
Respite Care
• 2 bed total (Male or Female) 1 bed for AREA 7 and 1 bed for AREA 10
• Males 10 to 18 years of age & Females 6 to 18 years of age at admission
• Contract is 4-7 days
Therapeutic Group Home Program
• 15 bed total capacity (Male or Female)
• Males 10 to 18 years of age & Females 10 to 18 years of age at admission
Intake and Placement Policy
Vera Lloyd Presbyterian Home and Family Services desires to handle client Intake and placement in a prompt, fair and consistent manner. This policy applies to all Department of Health and Human Services, United Family Services, Phoenix Youth and Family Services and Private Placement Clients. This placement and procedure list will be followed upon receipt of every referral for services by VLPH.
Intake Packet - Shelter-Residential Care
Intake Packet - Respite Care
A case representative contacts VLPH for services and availability of bed space is determined. The Admissions Coordinator must assess the client and determine placement appropriateness by the following:
• The client’s IQ must be 70 or above.
• The client must have no signs of Psychosis.
• The client cannot be a Sexual Perpetrator or Offender.
• The client must not be a Run Away Risk.
• The client must not be Physically Aggressive.
After assessment determines appropriateness of client, a VLPH intake packet is faxed or emailed to be completed in its entirety. The packet is to be faxed or email back to Admissions or accompany the client at the time of intake. When Respite services are rendered, a written form of approval must accompany the intake packet or be emailed by the contract financial coordinator. The proper home is contacted and given information regarding client and time of intake.
A case representative will accompany the client to the administration office for the intake process. The intake packet is completed, with all signatures and required paperwork being copied for Family-Teacher use. A photograph is taken of the client, along with the original paperwork. A file is set up by the VLPH office staff. The proper home is contacted and informed of the completion of the intake process. Client is accompanied by the case representative to the proper home and released to the Family-Teachers.
